A copy of your daughter's PHYSICAL EXAM and IMMUNIZATION RECORD needs to be turned in to the main school office. The physical exam should be taken between January 31-July 31, 2023.
If you should need it, the Cor Jesu Physical Exam Form can be used for documentation of a physical, but using it is not required. Printed documentation from your daughter's physical is also acceptable.
*The deadline for students taking a summer course for credit (PE I or Drawing I) is May 7, 2021. If you are enrolled in a summer course (PE I or Drawing I), your physical exam and immunization record must be on file by May 7, 2021. If needed, you may submit last year’s physical exam/immunizations by May 7th and then send in your 2021-2022 physical exam/immunizations by July 15th. Students will be notified by mid-March whether or not they were enrolled into their requested summer course.
