Health Information & Forms


Any student that intends to try out for a sport during the 2021-22 school year must complete the required health forms on PRIVIT by July 15* or they will not be allowed to try out:
  • MSHSAA Annual Pre-Participation Documentation Form – available on PRIVIT (Required every year)
  • MSHSAA Sports Physical Examination – upload to PRIVIT (SEE NOTE BELOW)
  • Athletic Transportation Form – available on PRIVIT

The Cor Jesu Athletic Department uses PRIVIT to collect the MSHSAA forms and the Cor Jesu Transportation form needed for students trying out for sports at Cor Jesu.  The directions to register for PRIVIT may be found here or by clicking on the sidebar link. PRIVIT has an app that will allow you to upload the MSHSAA Physical as needed.  Please remember that physicals are now good for two years unless indicated by your physician that it is only good for 1 year.  

All student athletes (both returning athletes and those wishing to try out for a team) must be registered on PRIVIT.   

Forms must be completed by July 15.
*For every day that the forms are late, a day will be added to the required number of practices for eligibility to play. If a student's forms are more than one week late, then she will no longer be allowed to try out.
NOTE:  The Physical Examination form is now valid for two years unless specified otherwise by a student's physician. However, the pre-participation form must be completed each year.
